Hyaluronic acid (HA) fillers can help to correct fine, medium and deep lines in the face and also to help plump or enhance the lips and areas of volume loss in the face. As we age, the face starts to droop—this is because we lose volume in the face (the ‘good’ fat goes way), the bones in the face thin out, and the supporting structures get lax. This translates into problem areas including jowls, ‘smoker lines’ around the mouth, depressed grooves along the sides of the mouth, and tired-looking eyes. We also lose lip volume, resulting in thinned lips and even thinned vermillion border, making the lip liner area less distinct. Hyaluronic acid filler injections can help to reverse the effects of time.

1)What should I know about filler injections?

We at the DeRosa Clinic recommend using hyaluronic acid fillers for the purpose of correcting fine lines and wrinkles in the face and also for adding volume to the face especially in the mid-face region. We naturally have hyaluronic acid in our bodies, so the body does not see this filler as a foreign object.
2) How do filler injections work?
Filler injections work by placing the filler with a thin needle underneath the skin in the region where those wrinkles or indentations are present. The HA filler itself is a gel that helps to fill in or lift up the deep wrinkles. Hyaluronic acid is also interesting because it draws water towards that region. So, you also get an additional fill and plumping from the effects of the water within your body coming to that site.
3) How should I plan my filler injections?
Our Team at the DeRosa Clinic will help you determine which fillers are best for you and how often you may need them. Hyaluronic acid fillers typically last about six to 12 months, however there are newer HA fillers such as Voluma XC that can last 18-24 months. In terms of downtime after filler injections, there can be some redness or swelling that may last a day or two. There is also always a small risk of bruising, which can be camouflaged using makeup. If you have a big event or a party where you want to look your best, we recommend having your filler injections done at least two weeks prior to that day.
4) What type of Fillers do you use in your Office?
Here in our office in Boston we use hyaluronic acid (HA) fillers. HA is a natural substance in the skin that delivers nutrients, helps the skin retain moisture and softness, and adds volume. We typically use Allergan’s line of fillers, and each product in the JUVÉDERM® collection of fillers is made from a modified form of HA. These include Juvéderm UltraXC, Juvéderm Ultra PlusXC, VolumaXC, Vollure XC, and Volbella XC. We also use Galderma’s Restylane HA Filler.
Fillers can be used for different applications. One application is to restore the volume in the mid part of the face. What happens as someone ages is that we lose the volume in the mid face. That volume droops, which also adds to deepen nasal labial folds, deepen lines on the lips, which we call drool lines, and this also adds to the jowling that can be seen in the lower third of the face.
5) What Is A Liquid Facelift?
A “liquid facelift” is a term used to describe using filler injections to restore facial volume and, when placed correctly, they can also lift the face to diminish laxity and jowls. This is what we call a liquid face lift, which one may hear about on the news. By restoring the volume into the mid-face, we’re also reversing the aging effects that you can see in the lower third of the face. The fillers used for this treatment typically last between six and 18 months, so it’s a great investment in your anti-aging process.
6) Can Fillers Help Reduce Circles Under My Eyes?
Filler injections can be used to help reduce the deepened circles, or what we call tear troughs, under the eyes. This is a great option for people who are starting to notice puffiness under the eyes and are considering whether or not they need or are ready for a lower lid eye lift procedure called a blepharoplasty. Instead of undergoing eyelid surgery, patients can have filler injections placed, which makes for a much simpler procedure with minimal to no downtime.
Sometimes patients will ask if fillers can be used in the crow’s feet area. Typically Botox® rather than fillers are used for crow’s feet, however sometimes we’ll use a combination of Botox® and filler injections for additional improvement. Botox® used in tandem with collagen induction treatment or microneedling also is a great option that can be used around the eye area to smooth out the crow’s feet and increase collagen and elastin production for the thin eyelid skin.
7) How will I know how to time my treatment?
If you’re considering filler injections and have a special occasion or event coming up, you may want to think about the timing for it and ideally schedule treatment at least 10 days to two weeks before the event. There is always the chance that some swelling and bruising may occur after your treatment. This is easily covered with makeup but it’s nice if you have an event to not have to worry about hiding small bruises.
It’s not typical for a patient to bruise to filler injections, but there are some things that can be done before the treatment to help reduce the risk. If not medically contraindicated, you can take Arnica pills or use Arnica gel on the area several days before treatment, as well as reduce or stop any blood thinners such as ibuprofen or aspirin (unless prescribed by your doctor). And just before the treatment in our office, we also provide you with ice to help reduce swelling and reduce the risk of bruising.
